Job Summary

A company is looking for a Telephonic Case Manager RN - Remote AZ.

Key Responsibilities

• Make outbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education
• Coordinate care for members and make referrals to outside sources

Required Qualifications

• Current unrestricted Multiple state licensure - Compact License
• Certified Case Manager (CCM) or ability to obtain certification within 18 months
• 3+ years of experience as a Registered Nurse in a hospital or telephonic Case Manager role
• Ability to navigate a Windows environment and utilize Microsoft Office applications
• Private home office and access to high-speed Internet
