Job Summary

A company is looking for a Telephonic Case Manager RN - Remote.

Key Responsibilities

• Make outbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education
• Coordinate care for members and make necessary referrals

Required Qualifications

• Current unrestricted Compact License
• 3+ years of experience as a Registered Nurse in a hospital or as a telephonic Case Manager
• Ability to obtain CCM certification within 18 months of employment
• Private home office and access to high-speed Internet
• Ability to navigate a Windows environment and utilize Microsoft Office applications
