Job Summary

A company is looking for a Transitional Care Associate.

Key Responsibilities

• Facilitates the timely discharge and transfer of clients from hospital care to post-acute settings
• Implements care plans in collaboration with the healthcare team and communicates transition arrangements
• Documents interventions in patient medical records and assists patients and families with post-acute care arrangements

Required Qualifications

• Bachelor's degree in social work or related field, or a Licensed Practical Nurse, or a Licensed Respiratory Therapist
• Knowledge of government/community agencies and resources, including Medicare and Medicaid
• Effective communication and customer service skills
• Ability to work flexible hours and weekends on rotation
• BLS certification required (not required for Insurance Division employees)
