Job Summary

Empowering patients toward optimal health, the full-time Arizona Licensed Virtually Integrated Care RN will provide comprehensive remote support, develop personalized care plans, and monitor patient progress while collaborating with healthcare teams in a dynamic virtual environment.

Key responsibilities

• Collaborates virtually with healthcare team members to assess and respond to patient needs, addressing routine, urgent, and emergent issues
• Oversees patient care plans to implement preventive measures and improve safety and quality of care delivered via telemedicine
• Monitors clinical operations and ensures compliance with internal policies and regulatory requirements while assessing patient outcomes and satisfaction

Required qualifications

• Bachelor's degree in Nursing
• Minimum of 3 years of overall acute care nursing experience, including 2 years of nursing preceptor experience
• 1-3 years in a nursing leadership role (Charge RN role applicable)
• Registered Nurse license in Arizona upon hire
• Basic Life Support (CPR) certification upon hire
