Job Summary

Empowering patients toward optimal health, the part-time Arizona Licensed Virtually Integrated Care RN will provide comprehensive remote support, develop personalized care plans, and collaborate with healthcare teams to enhance patient engagement and care coordination.

Key responsibilities

• Collaborates virtually with healthcare professionals to assess and address patients' medical, physical, and psychological needs
• Oversees patient care plans and implements preventive measures to improve safety and quality of care delivered via telemedicine
• Monitors clinical operations and ensures compliance with internal policies and regulatory requirements while assessing patient outcomes and satisfaction

Required qualifications

• Bachelor's degree in Nursing
• Minimum of 3 years of acute care nursing experience, with at least 2 years in a nursing preceptor role
• 1-3 years of experience in a nursing leadership role (Charge RN applicable)
• Registered Nurse license in Arizona upon hire
• Basic Life Support (CPR) certification upon hire
