Arizona or New Mexico Licensed LPN Care Manager

Job Summary

A company is looking for a Care Manager, LPN (AZ or NM License).

Key Responsibilities

• Perform comprehensive chart reviews, reconcile medications, and identify potential issues
• Develop and implement personalized care plans tailored to each patient's needs
• Monitor and assess patient progress, adjusting care plans as necessary

Required Qualifications

• Must be a Licensed Practical Nurse (LPN) in Arizona or New Mexico in good standing
• Proficient in using computers and various software applications
• Self-motivated and able to work independently in a remote setting
• Familiarity with chronic care management and geriatric care is preferred
• Previous experience in skilled nursing, assisted living, or long-term care settings is preferred
