Arizona Transition Specialist

Job Summary

A company is looking for a Transition Specialist to manage and coordinate the transition of members in the community.

Key Responsibilities

• Support care coordination teams to develop effective transition plans for members
• Assist with member transitions based on enrollment or care service needs
• Educate members and families on services and requirements related to transition planning

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Equivalent experience may be considered
• Knowledge of care coordination and transition services
• Experience in developing educational materials and training programs
