Arkansas Licensed Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers and ensure optimal healthcare outcomes
- Collaborate with providers to create ongoing service plans and connect members with necessary resources
- Monitor member progress and update care plans as needed, providing education and support to members and their families
Required Qualifications
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Registered Nurse (RN) state licensure or compact state licensure required
