Job Summary

A company is looking for a Case Manager, Registered Nurse (Remote).

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members
• Develop proactive care plans to enhance member wellness and address identified issues
• Collaborate with healthcare teams to overcome barriers and ensure comprehensive care

Required Qualifications, Training, and Education

• 3+ years of clinical practical experience
• 2+ years of care management, discharge planning, or home health care coordination experience
• Active and unrestricted Arkansas RN licensure and/or a compact license
• Associate's Degree in Nursing (REQUIRED); Bachelor's Degree (PREFERRED)
• Bilingual and Certified Case Manager preferred
