Job Summary

A company is looking for an RN Case Manager to provide comprehensive case management services remotely.

Key Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate health needs and services for individuals
• Act as a liaison among patients, families, healthcare providers, and community resources to ensure quality care and cost-effectiveness
• Ensure compliance with nursing regulations, policies, and standards while staying updated on healthcare trends and procedures

Required Qualifications

• Bachelor's degree in Nursing preferred
• Current active Registered Nurse (RN) license in good standing
• Case management certification required; must obtain URAC recognized certification within three years if not already certified
• Minimum of four years' clinical practice experience in relevant areas such as case management or critical care
• Proficiency in using Microsoft Office applications
