Associate Care Advocate

Job Summary

To support the development of innovative digital products, the full-time Associate Care Advocate will provide subject matter expertise on aging services, conduct consultations with older adults and caregivers, and collaborate with digital product teams to enhance user experiences, all while working remotely.

Key responsibilities

• Provide education and resources to aging individuals and caregivers navigating care options
• Conduct consultations and intensive case management, including crisis intervention
• Collaborate with product and design teams to report user needs and suggest improvements for digital products

Required qualifications

• Minimum of 5 years of relevant work experience in geriatric case management as a social worker or RN
• Knowledge of Area Agency on Aging resources, Medicaid and Medicare, and long-term care planning
• Experience applying qualitative and quantitative analysis to problem-solving
• Strong communication skills and ability to collaborate with diverse stakeholders
• Proven ability to deliver on responsibilities in a results-driven environment
