Job Summary

Key Responsibilities:

Respond to inquiries and educate policyholders and family members about the program
Conduct comprehensive assessments of policyholders and make program recommendations
Collaborate with the policyholder's care team and provide education on aging resources

Required Qualifications:

2-5 years of experience working with older adults and their families
College degree or certification/license in a related field
Experience with program enrollment geared towards seniors
Ability to effectively motivate individuals with post-operative musculoskeletal, neurological, and general acute problems
Knowledge of single and cumulative medical conditions common in the aging process
