Job Summary

To support post-acute care coordination, the full-time Bed Readiness Case Manager will manage patient care across various settings, optimize utilization of services, and enhance community partnerships while working remotely.

Key responsibilities
  • Manage Length of Stay (LOS) and rehospitalization risk
  • Coordinate cross-continuum care and drive bed readiness efforts
  • Develop and maintain community account relationships to support market growth
Required qualifications
  • Bachelor's degree in Healthcare Administration, Nursing, Social Work, Business, or related field; or equivalent experience
  • Three years of experience managing post-acute care resources within a Medicaid payer environment
  • Three years of experience coordinating care across various healthcare settings
  • Experience managing community-based provider relationships, including skilled nursing and assisted living facilities
  • Strong understanding of Medicaid regulations and care coordination models
