Behavioral Health Care Coordinator

Job Summary

Supporting the day-to-day functions of a complex medical network, the full-time Behavioral Health Care Coordinator will manage outreach, case assignments, and member education while working remotely.

Key responsibilities:

• Initiate outreach cycles and document member referral statuses within the electronic record
• Track and manage member needs, ensuring timely follow-up and coordination of healthcare services
• Provide education to members and caregivers regarding follow-up care, health status, and insurance benefits

Required qualifications:

• High School diploma required; Associate's degree in a related field preferred
• 3-5 years of experience in a professional environment required; healthcare experience preferred
• Proficiency in Microsoft Office applications and ability to learn MedHok, Casetrakker, or JIVA within 3 months
• Strong organizational skills and ability to manage multiple priorities
• Commitment to excellence in customer service and effective communication skills
