Job Summary

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

• Provide support to behavioral health clinical teams and facilitate member engagement
• Conduct outreach to educate members about specialty programs and assist with service location
• Collect patient progress information and handle various administrative duties related to care transitions

Required Qualifications

• 2 years of Behavioral Health care experience
• Previous customer service experience
• Proficiency with computer skills, including navigating multiple systems
• Associates Degree or equivalent combination of education and experience
