Job Summary

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

• Provide support to behavioral health clinical teams to enhance member engagement and care delivery
• Conduct outreach to members to educate them about specialty programs and assist with service location
• Collect patient progress information and facilitate follow-up calls while performing administrative duties

Required Qualifications

• 2 years of experience in Behavioral Health care
• Previous customer service experience
• Proficiency in computer skills, including navigating multiple systems
• Associates Degree or equivalent combination of education and experience
