Job Summary

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

• Provide comprehensive support to behavioral health clinical teams and facilitate member engagement
• Educate members about specialty programs and assist them in locating services
• Collect information on patient progress and perform miscellaneous administrative duties

Required Qualifications

• 2 years of Behavioral Health care experience
• Previous Customer Service experience
• Proficiency with computer skills, including navigating multiple systems
• Associates Degree or equivalent combination of education and experience
