Job Summary

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

• Provide comprehensive support to behavioral health clinical teams to enhance member engagement and care delivery
• Conduct outreach to members to educate them about specialty programs and facilitate their participation
• Assist members in locating services and collecting information on patient progress and treatment adherence

Required Qualifications

• 2 years of Behavioral Health care experience
• Previous customer service experience
• Proficiency with computer skills, including navigating multiple systems
• Associates Degree or equivalent combination of education and experience
