Job Summary

A company is looking for a Behavioral Health Utilization Care Manager in Arizona.

Key Responsibilities

• Manage individual members across the healthcare continuum to achieve optimal outcomes
• Evaluate medical necessity and appropriateness of care while optimizing patient outcomes
• Collaborate with healthcare teams to develop and manage interdisciplinary patient discharge plans

Required Qualifications, Training, and Education

• Master's degree in social work, counseling, or a related field with independent or associate licensure
• Licensure in social work, professional counseling, or marriage and family therapy (e.g., LCSW, LPC)
• Three years of clinical experience in counseling, care management, or case management
• Knowledge of care management, case management, and community resources
• Experience in behavioral health utilization management is preferred
