Job Summary

A company is looking for a Bereavement Coordinator (Remote) to support families and caregivers of deceased patients.

Key Responsibilities

Conduct Bereavement Assessments within 5 days of admission
Develop and implement a Bereavement Plan of Care for bereaved families
Provide bereavement support and education to hospice personnel and the IDG

Required Qualifications

Experience in counseling individuals and/or groups
Education or experience in grief or loss counseling
Preferred experience in hospice care
