Job Summary

To provide comprehensive patient care, the part-time Bi-Lingual RN Care Team will manage patient assessments, coordinate care plans, and collaborate with healthcare providers in a remote setting.

Key responsibilities

• Conduct patient assessments and develop individualized care plans
• Coordinate with healthcare providers to ensure continuity of care
• Educate patients and families on health management and resources

Required qualifications

• Current RN license in the state of practice
• Fluency in both English and another language
• Experience in patient care coordination or case management
• Strong understanding of healthcare regulations and standards
• Ability to work independently in a remote environment
