Job Summary

To enhance patient care, the remote Bi-Lingual RN Care Team will manage patient assessments, coordinate care plans, and provide education to clients across various locations.

Key responsibilities:

• Conduct patient assessments and develop individualized care plans
• Coordinate with healthcare providers and community resources to ensure comprehensive care
• Educate patients and families on health management and available services

Required qualifications:

• Active RN license with bilingual proficiency in English and another language
• Experience in care coordination or case management
• Strong understanding of healthcare systems and community resources
