Bilingual Advanced Practice Provider

Job Summary

To support a growing multidisciplinary team, the fully remote Bilingual Advanced Practice Provider will manage the clinical care of complex patients, utilizing telehealth visits and collaborating with care teams to ensure comprehensive support for patients with chronic conditions.

Key responsibilities

• Deliver exceptional care to complex patients through video telehealth visits and interdisciplinary collaboration
• Guide medical management using the 5M framework and perform comprehensive assessments, medication reviews, and quality of care evaluations
• Maintain accurate patient medical records and adhere to relevant legal and ethical guidelines

Required qualifications

• An active RN and APRN or Physician Assistant license in AZ, CO, HI, and/or OR at time of hire
• 3-5+ years of experience as an APP in primary care, geriatrics, or palliative care
• Proficiency in telehealth technology and electronic health records (EHR)
• Active BLS certification at time of hire
