Bilingual Advanced Practice Provider

Job Summary

To support a growing multidisciplinary team, the full-time Bilingual Advanced Practice Provider will deliver exceptional care to complex patients through telehealth visits, manage their clinical trajectories, and coordinate with interdisciplinary teams while working remotely.

Key responsibilities

• Deliver exceptional care to a cohort of complex patients leveraging video telehealth visits
• Guide and oversee the medical management of patients using the 5M framework
• Perform comprehensive assessments and coordinate care with the patient's support systems and care team

Required qualifications

• An active RN and APRN or Physician Assistant license with a willingness to obtain multiple state licenses
• Three or more years of experience as an APP in primary care, geriatrics, or palliative care
• Proficiency in telehealth technology and electronic health records (EHR)
• Active BLS certification at the time of hire
• Fluency in both Spanish and English is preferred
