Bilingual Advanced Practice Provider

Job Summary

To support a growing healthcare practice, the full-time Bilingual Advanced Practice Provider (NP or PA) will conduct virtual visits and manage prescription refills for patients with chronic conditions, utilizing digital tools and collaborating with a cross-functional care team, while working remotely from any of the contiguous 50 states.

Key responsibilities

• Conduct thorough assessments and develop individualized treatment plans for patients with chronic health conditions
• Manage a refill inbox for patients, ensuring adherence to established protocols and guidelines
• Participate in centralized results management workflows, reviewing and triaging lab and diagnostic results in a timely manner

Required qualifications

• Bilingual in Spanish and English
• Active and in-good-standing RN and APRN licenses for Nurse Practitioners or an active and in-good-standing Physician Assistant license
• A minimum of three years of outpatient clinical practice experience, with a focus on hypertension and lipid management
• Experience in primary care or a subspecialty with a strong emphasis on chronic disease management
• Ability to obtain licensure in requested states within 90 days of hire
