Job Summary

To deliver exceptional preventive cardiovascular care, the full-time Bilingual Cardiometabolic APP will conduct virtual consultations for patients with hypertension, collaborate with an interdisciplinary team, and utilize advanced technology to optimize patient management in a remote setting.

Key responsibilities

• Conduct thorough assessments and develop individualized treatment plans for patients with hypertension through virtual consultations
• Collaborate with an interdisciplinary care team to coordinate holistic, patient-centered care and ensure continuity of treatment
• Utilize electronic health information systems to manage patient records and provide feedback for system improvements

Required qualifications

• Active RN and APRN or Physician Assistant license with a willingness to obtain and maintain multiple state licenses
• 5+ years of outpatient clinical practice, including 2+ years in primary care or a relevant subspecialty focused on hypertension management
• Fully bilingual in Spanish and English
• Ability to work Monday-Friday from 8:30am-5:00pm MST or PST
• Strong clinical foundation in primary and secondary prevention of atherosclerotic cardiovascular disease
