Bilingual Care Advisor

Job Summary

A company is looking for a Care Advisor, Care Concierge - Bilingual (English/Spanish).

Key Responsibilities

• Assist members with assessments and recommend service plans based on their needs
• Make outbound calls for follow-up and support resource access for members and families
• Coordinate and schedule services while performing administrative tasks related to member benefits

Required Qualifications

• High School Diploma preferred
• Experience in Public/Community Health or related fields preferred
• Experience in Social Services, Care Coordination, or Healthcare Navigation is a plus
• Must be 18 years of age or older
• Computer proficiency and knowledge of clerical functions are required
