Job Summary

A company is looking for a Bilingual Care Advocate to assist seniors, caregivers, and families in navigating the healthcare system.

Key Responsibilities

• Establish trusting relationships with members through active listening and empathy
• Conduct comprehensive assessments to identify barriers to care
• Develop and execute individualized care plans integrating medical and social determinants of health

Required Qualifications

• Bilingual fluency in Spanish and English
• 2+ years of experience in care management, patient advocacy, or clinical social work
• Deep understanding of Social Determinants of Health (SDOH) and experience supporting diverse communities
• Highly organized with the ability to manage a caseload and prioritize tasks
• Comfortable navigating EMRs and care-management software
