Job Summary

A company is looking for a Care Coordinator, Bilingual Spanish.

Key Responsibilities

• Serve as the primary point of contact for families, managing logistics of care and developing comprehensive care plans
• Coordinate medical appointments, research providers, and assist with insurance and facility vetting
• Collaborate with Care Advisers to ensure high-quality service and support for families throughout the caregiving process

Required Qualifications

• Bachelor's degree in Social Work, Public Health, Healthcare Administration, or a related field; advanced degrees are a plus
• Bilingual fluency in Spanish (reading, writing, and speaking)
• 4-6+ years of healthcare experience, specifically in case management or crisis intervention
• Experience in developing and facilitating care plans
• Ability to manage up to 40 care projects with compassion and efficiency
