Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator (Fully Remote, Spanish Speaking).

Key Responsibilities

• Manage a defined caseload of enrolled Chronic Care Management (CCM) members
• Conduct proactive outreach to support ongoing engagement and care coordination
• Coordinate referrals to community-based organizations and social service providers

Required Qualifications

• Minimum of 1 year of experience in care coordination, community health work, or a related member-facing support role
• Bilingual - Spanish and English fluency required
• At least 1 year of experience working with a Medicare/Medicaid population
• Experience supporting individuals with chronic conditions and ongoing care needs
• Strong understanding of social determinants of health and community-based resources
