Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator for Outpatient Case Management.

Key Responsibilities

• Assist members with referrals, authorizations, and scheduling appointments
• Create cases and complete documentation for hospital and skilled nursing facility discharges
• Collaborate with the case manager to manage a panel of members and update them on changes in condition

Required Qualifications, Training, and Education

• Minimum of 1 year experience in health care or related field
• High School Diploma or GED required; Bachelor's degree or equivalent experience preferred
• Bilingual in English and Spanish preferred
• Medical assistant training or medical terminology training preferred
• Basic computer proficiency and knowledge of managed care plans and Medi-Cal
