Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator, Transition of Care.

Key Responsibilities

• Create cases and complete documentation for hospital and SNF discharges
• Assist members with referrals, authorizations, and scheduling provider appointments
• Collaborate with the RN Case Manager to manage member care and update records

Required Qualifications, Training, and Education

• Minimum 1 year of experience in care coordination or case management
• Experience with scheduling, authorizations, and coordination of services
• High School Diploma or GED required; Bachelor's degree preferred
• Bilingual in English and Spanish
• Medical assistant training or medical terminology training preferred
