Bilingual Care Coordinator (NM)

Job Summary

To support patient care initiatives, the full-time Bilingual Care Coordinator (NM) will manage a panel of 400-450 patients, providing health coaching and conducting monthly outreach from a remote, HIPAA-compliant workspace.

Key responsibilities

• Conduct 50-60 daily outbound calls to patients, offering care plan support and health coaching
• Collaborate with clinical teams to identify and address patients' clinical and social needs
• Advocate for patients by actively listening to their concerns and communicating solutions to their care circle

Required qualifications

• Bilingual in Spanish and English, with fluency in both written and spoken communication
• Nationally recognized medical assistant certification or Certified Pharmacy Technician certification
• Minimum of 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum internet speed requirements
