Bilingual Care Coordinator (NM Licensed)

Job Summary

To support patient care initiatives, the full-time Bilingual Care Coordinator (NM Licensed) will manage a panel of 400-450 patients, conduct monthly outreach, and collaborate with clinical teams, all while working remotely from a HIPAA-compliant home office.

Key responsibilities

• Conduct 50-60 daily outbound calls to provide health coaching and care plan support to patients
• Collaborate with clinical teams to identify and address patient needs and close gaps in care
• Document all patient interactions in real time while maintaining HIPAA compliance

Required qualifications

• Bilingual in Spanish and English, with fluency in both written and spoken communication
• Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician certification
• A minimum of 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
