Bilingual Care Guide

Job Summary

Supporting patients and their caregivers, the full-time Bilingual Care Guide will proactively identify and resolve practical, social, physical, and emotional issues while working remotely to enhance the patient experience.

Key responsibilities

• Develop relationships with patients through telephonic outreach following clinical events or diagnoses
• Collaborate with the clinical team and utilize community resources to improve patient outcomes
• Empower patients to communicate their preferences and actively participate in their healthcare decisions

Required qualifications

• Bilingual in Spanish and English
• Proficiency with a personal computer and experience with Microsoft Excel preferred
• Bachelor's degree preferred, or equivalent professional experience considered
• Technical and analytical skill set
• Excellent organizational and interpersonal skills
