Key Responsibilities

• Engage with members to navigate their cancer journey and identify health and social needs
• Connect members to healthcare and community resources, including advanced care planning support
• Handle inbound and outbound calls in a structured, high-volume environment while managing documentation and coordination tasks

Required Qualifications

• At least 2 years of experience in a patient-facing role, such as care coordination or healthcare navigation
• Bilingual proficiency in Spanish and English
• Experience in a fast-paced, metrics-driven environment
• Ability to build rapport quickly with members in a primarily phone-based setting
• Comfort with technology and remote work environments
