Bilingual Care Partner

Job Summary

As a bilingual Spanish/English Care Partner, the full-time position will engage members in navigating their cancer journey through phone, text, and email, while connecting them to healthcare and community resources in a remote environment.

Key responsibilities

• Engage with members to assess their health and social needs, providing support and resources for advanced care planning
• Serve as the primary point of contact for members, coordinating care and connecting them with appropriate healthcare providers and community resources
• Manage high-volume inbound and outbound calls, ensuring timely documentation and adherence to productivity metrics

Required qualifications

• At least 2 years of experience in a patient-facing role such as care coordination or healthcare navigation
• Bilingual proficiency in Spanish and English
• Experience in a fast-paced, metrics-driven environment, such as a contact center
• Ability to assess and address both expressed and unexpressed needs of members
• Comfort with technology and experience using electronic health records or similar systems
