Bilingual Case Management Nurse (RN)

Job Summary

To support comprehensive patient-centered care, the full-time Bilingual Case Management Nurse (RN) will assess patient needs, develop individualized care plans, and advocate for patients while working remotely.

Key responsibilities:

• Coordinate and manage healthcare services to ensure effective communication among providers
• Assess patients' medical needs and develop individualized care plans aligned with their health goals
• Monitor patient progress and adherence to care plans, adjusting interventions as necessary

Required qualifications:

• Active, unrestricted Registered Nurse (RN) license in state of residence; Compact License (NLC) accepted
• 3+ years of nursing experience in an acute care setting
• Fluency in both English and Spanish, verbally and in writing
• Demonstrated critical thinking and sound judgment in complex situations
• Excellent communication and interpersonal skills with a collaborative approach
