Bilingual Community Case Manager

Job Summary

A company is looking for a Bilingual Community Guide Case Manager.

Key Responsibilities

• Establish trust and build strong relationships with members
• Create personalized care plans tailored to members' needs
• Connect members to community services and follow up to ensure services were received

Required Qualifications and Education

• Minimum of 2 years in case management or care coordination
• Bilingual in English and Spanish or English and Haitian-Creole
• Experience in health insurance or healthcare, preferably Medicare Advantage
• Ability to support either Mountain or Pacific time zones
• Advanced Degree in Social Work or mental health/psychology preferred
