Job Summary

A company is looking for a Bilingual Continuous Care Coach to manage the care needs of patients with chronic eye diseases.

Key Responsibilities

• Manage patient caseload and communicate based on patient needs
• Educate patients and families on self-management and treatment adherence
• Develop and maintain personalized care plans in collaboration with healthcare providers

Required Qualifications

• Bilingual in English and Spanish
• 2+ years of work experience in eye care
• Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) required
• Proficient with Electronic Medical Record (EMR) systems, preferred
• Ability to work independently in a remote environment
