Job Summary

Providing telephonic coaching and educational resources, the full-time salaried Bilingual Disease Management Coach - RN will assist individuals with chronic health conditions in managing their care, focusing on medication compliance, nutrition, and overall wellness in a 100% remote environment.

Key responsibilities

• Deliver telephonic coaching and referral services to participants managing chronic health conditions
• Assess participant needs and coordinate care with healthcare professionals
• Document coaching sessions and participant activities in case tracking software

Required qualifications

• Nursing Degree from an accredited college or university
• Active and unrestricted Compact Registered Nurse License
• 2+ years of experience in direct participant coaching
• Intermediate knowledge of Microsoft Office and database software
