Bilingual Home Health Coordinator

Job Summary

A company is looking for a Bilingual Home Health Referral Care Coordinator.

Key Responsibilities

• Provide staffing for home health and IV therapy referrals and assist with new home health referrals
• Ensure accuracy of demographic data and obtain necessary documentation for case staffing
• Manage phone calls, document management, intake referrals, and pricing negotiations

Required Qualifications

• Some college education and/or medical background; bilingual in Spanish required
• Experience with Work Comp insurance, referrals, and authorizations preferred
• Knowledge of medical terminology and moderate PC skills, including MS Office
• Able to work cohesively with coworkers and external customers with minimal supervision
• Knowledgeable of injured workers' rights and ensure confidentiality of all data
