Job Summary

A company is looking for a Bilingual LPN Care Navigator.

Key Responsibilities

• Engage in monthly telephonic patient interactions to support care goals
• Create individualized care plans and manage coordination via proprietary software
• Document interactions and escalate clinical concerns as necessary

Required Qualifications

• Bilingual (Spanish/English) - fluent in both written and spoken communication
• Active Licensed Practical Nurse or Licensed Vocational Nurse license with multi-state compact license
• Minimum 2 years' clinical experience
• Experience working with EMR systems and familiarity with chronic condition management
• Intermediate to advanced computer skills and telephonic communication skills
