Bilingual Palliative Care NP/PA

Job Summary

A company is looking for a Bilingual (Spanish) Advanced Practice Provider (NP or PA) in Palliative Care.

Key Responsibilities

• Manage a panel of high-risk patients through assessments and development of Palliative Plans of Care
• Conduct regular telephonic and/or video encounters for ongoing patient care and support transitions
• Collaborate with interprofessional teams and participate in required meetings and activities to ensure seamless patient care

Required Qualifications, Training, and Education

• An unrestricted advanced practice license in one or more specified states
• Minimum of 5 years in advanced practice, with at least 3 years in Palliative care
• ACHPN credential or willingness to obtain within the first six months of employment
• Comfortable with technology and a fast-paced work-from-home environment
• Willingness to obtain additional state licenses as requested within 90 days
