Bilingual Palliative Care NP/PA

Job Summary

A company is looking for a Bilingual (Spanish) Advanced Practice Provider (NP or PA) specializing in Palliative Care.

Key Responsibilities

• Manage a panel of high-risk patients through regular telephonic and/or video encounters
• Perform initial assessments and develop Palliative Plans of Care for newly enrolled members
• Collaborate with interprofessional teams to coordinate care and support transitions for patients

Required Qualifications

• Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state
• Minimum of 5 years in advanced practice, with at least 3 years in Palliative care
• ACHPN credential or willingness to obtain within the first six months of employment
• Bilingual in English and Spanish
• Comfortable with technology and working in a fast-paced environment
