Job Summary

A company is looking for a Bilingual Registered Nurse - CCM.

Key Responsibilities

• Continuously monitor patient vitals, symptoms, and lab results to identify care gaps
• Create and manage personalized care plans aligned with treatment goals
• Conduct virtual follow-up appointments and document clinical interactions

Required Qualifications

• Active Multi-State RN Compact License and a single state RN license in NY
• Bilingual in English and Spanish
• Associate Degree in Nursing Science
• 5+ years of recent clinical experience as an RN in an ICU/ER setting
• Prior experience in chronic care management is a plus
