Bilingual Remote Care Coordinator - LPN

Job Summary

A company is looking for a Full-Time Bilingual Remote Care Coordinator - CMA/RMA or LPN.

Key Responsibilities

• Conduct monthly CCM phone calls with assigned patients
• Make 30-50 outbound calls per day and complete required patient interactions
• Coordinate care with provider offices, pharmacies, and community resources while maintaining HIPAA compliance

Required Qualifications

• Minimum 1-3 years of experience as a CMA/RMA, LPN, or RN
• Active certification or multistate license for LPN/RN
• Experience with EMR/EHR systems
• Comfortable being on the phone most of the day
• Organized, reliable, and able to work independently in a remote setting
