Bilingual RN Care Coach

Job Summary

Working remotely on a full-time basis, the Bilingual RN Care Coach will manage the initial assessment process for families, guide caregivers through service activation, and develop personalized care plans while advocating for caregiver and consumer health needs.

Key responsibilities

• Serves as the initial contact for the coaching team, providing information and guiding caregivers through the referral process
• Coordinates and completes all applicable assessments and ensures compliance with activation procedures
• Develops and reviews ongoing care plans while providing clinical consultation and education to the coaching team

Required qualifications

• Active Registered Nurse (RN) License in the state(s) of employment
• Minimum two (2) years of recent experience in direct care of elders or adults with disabilities
• Experience in case management, care planning, and assessment in a healthcare setting
• Fluency in both English and Spanish is required
• Proficiency in common software and web-based applications (e.g., MS Office, case management systems)
