Bilingual RN Care Manager

Job Summary

A company is looking for a Bilingual RN Care Manager (Remote Flexible - Spanish Speaking).

Key Responsibilities:

• Work with individuals with complex medical needs to identify health/wellness goals and incorporate them into a Shared Care Plan
• Educate individuals on medical and behavioral health conditions, provide care management services, and track required assessments and screenings
• Collaborate with a multidisciplinary care team to address barriers to care and engage individuals in care using relationship-based strategies

Required Qualifications:

• Active Registered Nurse license issued by the state of California
• 5+ years of experience in care coordination or case management
• Bilingual - English/Spanish
• Strong technical skills and comfort with new technology innovation
• Understanding of cultural fluency and experience working with underserved communities
