Job Summary

A company is looking for a Care Management Registered Nurse with RN Compact and NY License, and bilingual in Spanish.

Key Responsibilities

• Continuously monitor patient vitals, symptoms, and lab results to identify care gaps
• Create and manage personalized care plans aligned with treatment goals
• Conduct virtual follow-up appointments to support patient enrollment and adherence

Required Qualifications, Training, and Education

• Multi-state RN Compact State Licensure and license in NY
• Associate Degree in Nursing Science
• 5+ years of clinical experience in a chronic care management program
• Skilled in nursing processes and clinical acumen
• EMR experience, preferably in Athena and EPIC
